Domestic Patient Estimates
THIS IS AN ESTIMATE. Guest estimates will create estimates with expected out-of-pocket costs based on your expected service and your specific insurance co-pay, co-insurance and deductible information. Your final out-of-pocket financial responsibility may vary from this estimate if additional services are performed, your medical condition changes, or complications occur. You will be billed for all services based on your needs and medical condition. Additional services not listed in this estimate may be performed to better care for you. Also, services performed by a physician not employed by Henry Ford Health are billed separately and are not included in this estimate. If you provide insurance information, your out-of-pocket responsibility will be determined based on information we receive from your insurance provider.
